      摘要:Based on the core pathogenesis of qi deficiency and blood stasis in cirrhosis after hepatitis, the theory of traditional Chinese medicine (TCM) pathogenesis of liver cirrhosis “blood stasis is the superficial sign of lump accumulation, and intrinsic deficiency is the root cause of lump accumulation” is proposed and discussed. Studies using multiple liver cirrhosis models under intervention of classical TCM formulas with different function and effects have shown that the role of removing stasis by Xiayuxue Decoction against liver cirrhosis is characterized by targeting abnormal deposition of fibrous connective tissue, hepatic sinus capillarization and abnormal angiogenesis in the liver. The mechanism of Xiayuxue Decoction includes downregulating the expression of tissue inhibitors of metalloproteinases, increasing the activities of matrix metalloproteinases, promoting the degradation of pathological deposited collagens, inhibiting the activation of hepatic stellate cells and inducing their apoptosis, improving the hepatic sinus capillarization and inhibiting angiogenesis, which are its charcteristics of “treating symptoms”. Analyzing the experimental results related to “disease-formula (effect)-syndrome” reveals that the key pathological basis of the blood stasis in liver cirrhosis is mainly abnormal deposition of liver fibrous connective tissue and proliferation of activated stellate cells, low collagen degradation activity, lack of liver sinusoidal endothelial cell fenestration and formation of subendothelial basement membrane, and increased capillarization. The above provides a reference for clarifying the disease indications (pathological indications) targeted by stasis-dispelling formulas (such as Xiayuxue Decoction), and helps to promote clinical translational application.

      摘要:Objective: To explore the therapeutic effect of acupuncture on negative symptoms in patients with schizophrenia and provide the evidence for clinical treatment.MethodsA total of 100 schizophrenic patients with negative symptoms were included and divided into the study group and control group by random number table, 50 cases in each group. The patients in both groups were orally treated with amsulpiride tablets. Based on above, the patients in the study group were treated with acupuncture. The observation period was 12 weeks. Before and after treatment, the positive and negative syndrome scale (PANSS) and scale for assessment of negative symptoms (SANS) were applied to evaluate the improvement of negative symptoms of schizophrenia in both groups.ResultsDuring the study, 4 patients in the study group were removed or shed off, and 46 patients in the study group and 50 patients in the control group were finally included in the statistical analysis. After treatment, the negative symptom, positive symptom, general psychopathological symptom scores and total score of PANSS in the study group were significantly decreased compared with those before treatment (P<0.01), the negative symptom score in the control group was also decreased (P<0.05), and the negative symptom, general psychopathological symptom scores and total score of the study group were significantly lower than those of the control group (P<0.01). After treatment, the scores of all items of SANS in the study group were significantly decreased compared with those before treatment (P<0.01), while there were no statistically significant differences on the scores of all items in the control group between before and after treatment (P>0.05), and the scores of all items in the study group were lower than those in the control group (P<0.05, P<0.01).ConclusionCompared with antipsychotic drugs alone, acupuncture combined with antipsychotic drugs can better improve the negative symptoms of patients with schizophrenia, which has certain clinical application value.

      摘要:Objective: To investigate the clinical efficacy of Shenshuaiyi Formula in the treatment of chronic kidney disease (CKD) at stage 4 combined with chronic heart failure (CHF).MethodsEighty patients of CKD at stage 4 combined with CHF (spleen and kidney qi deficiency and blood stasis syndrome) were randomly divided into the control group and the treatment group, 40 cases in each group. The patients in the control group were treated with routine western medicine, and the patients in the treatment group were treated with routine western medicine combined with Shenshuaiyi Formula orally, with a course of 8 weeks. The clinical efficacy of the two groups was evaluated. Before and after treatment, the changes of the total scores of traditional Chinese medicine (TCM) syndrome and the main symptom scores were compared between the two groups, and the levels of serum creatinine (SCr), urea nitrogen (BUN), estimated glomerular filtration rate (eGFR), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), hypersensitive C-reactive protein (hsCRP) and N-terminal B-type brain natriuretic peptide precursor (NT-proBNP) of all the patietns were detected, the left ventricular ejection fraction (LVEF) was compared between the two groups, and the New York Heart Association (NYHA) cardiac function grading of the patients in the two groups was evaluated.ResultsDuring the course of treatment, 1 patient in the control group was removed and 1 patient in the treatment group was shed off, finally 39 cases in the control group and 39 cases in the treatment group were included in the statistical analysis. ①After treatment, the total clinical effective rate was 84.62% in the treatment group and 56.41% in the control group, and the effect of the treatment group was better than that of the control group (P<0.05). ②After treatment, the total scores of TCM syndrome in the two groups were significantly decreased (P<0.05), and the score of the treatment group was lower than that of the control group (P<0.05). The main symptom scores in the two groups were decreased compared with those before treatment (P<0.05), and the scores of fatigue, shortness of breath and taciturnity, lack of appetite, and aching and weakness of waist and knee symptoms in the treatment group were lower than those in the control group (P<0.05). ③After treatment, the levels of SCr, BUN, TNF-α, IL-6, hsCRP and NT-proBNP in the two groups were decreased compared with those before treatment (P<0.05), the levels of eGFR and LVEF were increased compared with those before treatment (P<0.05), and the levels of SCr, BUN, TNF-α, IL-6, hsCRP and NT-proBNP in the treatment group were lower than those in the control group (P<0.05), the levels of eGFR and LVEF were higher than those in the control group (P<0.05). ④After treatment, the NYHA cardiac function grading of the patients in the two groups was improved (P<0.05), and the improvement of the treatment group was better than that of the control group (P<0.05).ConclusionShenshuaiyi Formula combined with conventional western medicine can better improve the renal and cardiac functions of the patients with CKD at stage 4 combined with CHF (spleen and kidney qi deficiency and blood stasis syndrome), decrease the levels of serum inflammatory factors, and enhance the clinical efficacy.

      摘要:Objective: To investigate the effects of Jianpi Qinghua Formula on the blood glucose fluctuation and the skeletal muscle content and function in patients of initial type 2 diabetes mellitus (T2DM) with type of qi and yin deficiency.MethodsSeventy patients of initial T2DM with type of qi and yin deficiency were included and randomly divided into the treatment group and control group, 35 cases in each group. The patients in the two groups were given lifestyle intervention. Based on above, the patients in the treatment group were orally treated with Jianpi Qinghua Formula, and the patients in the control group were orally treated with placebo, with a course of 12 weeks. After treatment, the clinical efficacy of the two groups were evaluated and compared. Before and after treatment, the total scores of traditional Chinese medicine (TCM) syndrome were compared between the two groups, and the levels of the time in the target range of blood glucose (TIR), the skeletal muscle mass of extremities, the dominant hand grip strength, and glycosylated hemoglobin (HbA1c), fasting blood glucose (FBG), 2-hour postprandial blood glucose (2 h PBG) were detected.ResultsIn the course of research, 2 patients in the treatment group and 1 patient in the control group were shed off or removed, finally 33 cases in the treatment group and 34 cases in the control group were included in the statistical analysis. ①After treatment, the total clinical effective rate was 75.8% in the treatment group and 38.2% in the control group, and the therapeutic effect of the treatment group was better than that of the control group (P<0.01). ②After treatment, the total score of TCM syndrome in the treatment group was decreased compared with that before treatment (P<0.05), and the improvement of the total score of TCM syndrome in the treatment group was better than that in the control group (P<0.05). ③After treatment, the TIR of the treatment group showed an upward trend, but there was no statistically significant difference between before and after treatment (P>0.05), the TIR of the control group was significantly decreased (P<0.05), and the improvement of TIR in the treatment group was better than that in the control group (P<0.05). ④After treatment, the skeletal muscle mass of extremities and dominant hand grip strength in the treatment group were increased (P<0.05), while the skeletal muscle mass of extremities and dominant hand grip strength in the control group were decreased (P<0.05), and the improvement of skeletal muscle mass of extremities and dominant hand grip strength in the treatment group was superior to those in the control group (P<0.05). ⑤After treatment, the level of HbA1c in the treatment group was decreased (P<0.05), and the improvement of HbA1c level in the treatment group was superior to that in the control group (P<0.05).ConclusionJianpi Qinghua Formula can improve the blood glucose fluctuation, increase the dominant hand grip strength and extremity skeletal muscle content, and improve the glucose metabolism and clinical symptoms in patients of initial T2DM with type of qi and yin deficiency. Its mechanism of improving blood glucose fluctuation may be related to improving the skeletal muscle mass and function.

      摘要:Objective: To observe the clinical efficacy of Maiqi Yifei Mixture on chronic cough in children.MethodsSeventy-two children with chronic cough were randomly divided into observation group (36 cases) and control group (36 cases). Both groups were treated with routine western medicine, and the observation group was additionally treated with tradtional Chinese medicine Maiqi Yifei Mixture. Treatment in the two groups lasted for 4 weeks. The clinical efficacy of the two groups was compared through the time of relief of cough symptoms, the time of disappearance of cough symptoms, and the changes of serum total immunoglobulin E (IgE), eosinophil (EOS) and other indexes in the two groups.Results① After treatment, the scores of TCM main symptoms in both groups were decreased (P<0.05). ②After treatment, the time of relief of cough symptoms and the time of disappearance of cough symptoms in the observation group were shorter than those in the control group (P<0.05). ③ After treatment, the improvement of serum total IgE and eosinophil (EOS) in the observation was better than that in the control group (P<0.05).ConclusionIn treating chronic cough in children, Maiqi Yifei Mixture can effectively improve the cough symptoms and the clinical efficacy.

      摘要:Objective: To observe the effect of Lujiao Zhuangdu Decoction on inflammatory factors of ankylosing spondylitis (AS) and its clinical efficacy.MethodsA total of 80 AS patients were randomly divided into the control group and the treatment group,40 cases in each group. The control group was given oral celecoxib capsules, and the treatment group was given Lujiao Zhuangdu Decoction on the basis of the control group, and the course of treatment was 12 weeks. After the course of treatment, the clinical efficacy of the two groups and the levels of serum inflammatory indexes CRP,ESR,IL-6,TNF-α,ankylosing spondylitis functional index(BASFI),ankylosing spondylitis disease activity index(BASDAI),ankylosing spondylitis metrology index(BASMI), ankylosing spondylitis disease activity score(ASDAS),spinal range of motion (Schober) test,finger-to-ground distance,pillow-to-wall distance,and changes in thoracic expansion measurements were observed.Results①Finally,40 cases in the experimental treatment group and 39 cases in the control group were completed.②The total effective rate in the treatment group was 90.00%,and the total effective rate in the control group was 84.62%; the difference in clinical efficacy between the two groups was statistically significant (P<0.05).③After treatment, the CRP,ESR,IL-6 and TNF-α in the two groups were lower than those before treatment(P<0.05).Compared with the two groups after treatment,CRP,ESR,IL-6 and TNF-α in the treatment group were significantly lower than those in the control group (P<0.05).④After treatment, the BASFI,BASDAI, and BASMI scores of the two groups were all lower than those before treatment(P<0.05). Compared with the two groups after treatment, the BASFI,BASDAI, and BASMI scores in the treatment group were significantly lower than those in the control group (P<0.05).⑤After treatment, the scores of ASDAS-CRP and ASDAS-ESR in the two groups were decreased compared with those before treatment(P<0.05).Compared with the two groups after treatment, the scores of ASDAS-CRP and ASDAS-ESR in the treatment group were significantly lower than those in the control group(P<0.05).⑥After treatment, the measured values of Schober’s test in the two groups were increased and the finger-to-ground distance were decreased compared with those before treatment (P<0.05).Compared with the two groups after treatment, the measured values of Schober’s test and finger-to-ground distance in the treatment group were better than those in the control group (P<0.05);pillow-wall distance and thoracic expansion were not significantly improved within or between groups(P>0.05).ConclusionLujiao Zhuangdu Decoction can effectively down-regulate the level of inflammatory factors in AS patients, reduce BASFI,BASDAI,BASMI,ASDAS-CRP,ASDAS-ESR scores, and improve Schober test and finger-to-ground distance with significant curative effect and safety.

      摘要:Objective: To observe the clinical efficacy of Shimi Formula in treating perioperative patients of mixed hemorrhoids with constipation and its influence on the levels of serum motilin (MTL), vasoactive intestinal peptide (VIP) and substance P (SP).MethodsNinety-six patients of mixed hemorrhoids with constipation were included and divided into the Chinese medicine group (48 cases) and western medicine group (48 cases) according to the random number table. The patients in the Chinese medicine group were orally treated with Shimi Formula and the patients in the western medicine group were orally treated with lactulose. After 7 days of medication, all the patients were given Milligan-Morgan hemorrhoidectomy and continued to take medication for 4 weeks after surgery. Before medication and after 7, 14, 21 and 35 days of medication, the scores of defecation condition indexes of the two groups were evaluated and compared. Before medication and after 14 and 35 days of medication, the anxiety and depression of the patients were evaluated by self-rating anxiety scale (SAS) and self-rating depression scale (SDS). On the day and the 3rd, 7th and 14th days after surgery, the scores of postoperative complication (including pain, bleeding, edema, and urinary retention) in the two groups were evaluated and compared. The wound healing time of the two groups were recorded and compared. Before medication and after 35 days of medication, the levels of serum MTL, VIP and SP were detected. After 35 days of medication, the clinical efficacy of the two groups was compared.Results①After medication, the defecation condition of the two groups were significantly improved compared with that before medication (P<0.05, P<0.01), and the improvement of the Chinese medicine group was obviously superior to the western medicine group (P<0.05, P<0.01). ②After medication, the scores of SAS and SDS in the two groups were significantly decreased compared with those before medication (P<0.01), and the scores of the Chinese medicine group were obviously lower than those of the western medicine group (P<0.01). ③After surgery, the scores of pain, bleeding and edema in the Chinese medicine group were lower than those in the western medicine group (P<0.05, P<0.01). ④There was no statistically significant difference on the wound healing time between the two groups (P>0.05). ⑤After 35 days of medication, the levels of MTL and VIP in the two groups were significantly decreased (P<0.01), the level of SP was significantly increased (P<0.05), and the levels of MTL and VIP in the Chinese medicine group were lower than those in the western medicine group (P<0.05), the level of SP was higher than that in the western medicine group (P<0.05). ⑥After medication, the total clinical effective rate was 87.50% in the Chinese medicine group and 66.67% in the western medicine group, and the effect of the Chinese medicine group was superior to the western medicine group (P<0.05).ConclusionShimi Formula can significantly relieve the defecation difficulty, alliviate the anxiety and depression, reduce the postoperative complications, and regulate the serum gastrointestinal hormone levels in treating perioperative patients of mixed hemorrhoids with constipation, which has better therapeutic effects.

      摘要:Objective: To investigate the chemical structure and purity determination method of momordica saponin Ⅰ (MSⅠ).Methods2D NMR technology and hydrolysis method were used to determine the chemical structure and assign the NMR data. 1H-NMR technology was used to determine the purity.ResultsThe chemical structure of MSⅠ was rapidly determined and the NMR data was completely assigned for the first time by 2D NMR technology combined with acid and alkaline hydrolysis. A simple, rapid, and accurate method for the purity determination of MSⅠ was established by 1H-NMR which didn’t need specific reference substance.ConclusionThe study provided a methodological reference for the chemical structure and purity determination of complex saponins.

      摘要:Objective: To explore the mechanism of total flavonoids from Clerodendranthus Spicatus on the apoptosis of human renal clear cell carcinoma 786-O cells.MethodsHuman renal clear cell carcinoma 786-O cells were treated with different concentrations (0, 25, 50, 100, 200, 400 µg/mL) of total flavonoids from Clerodendranthus Spicatus. Cell viability was detected by CCK-8 assay, and cell apoptosis was measured by Annexin V/propidium iodide (PI) staining. RNA-seq library sequencing analysis was carried out through Illumina Novaseq 6000 platform on cells treated with 0 and 139.9 µg/mL of total flavonoids from Clerodendranthus Spicatu. Pathway-related genes were obtained and verified by RT-qPCR and Western blot.Results①CCK-8 results showed that the viability of human renal clear cell carcinoma 786-O cells decreased in a time and concentration-dependent manner after treated with total flavonoids from Clerodendranthus Spicatu. And total flavonoids from Clerodendranthus Spicatu induced apoptosis of 786-O cells in a concentration-dependent manner. ②A total of 2 047 differential genes were identified by transcriptome sequencing, among which 773 were up-regulated and 1 274 were down-regulated. Gene Ontology (GO) enrichment analysis showed that the differential genes were mainly involved in processes such as cellular stress response, apoptotic signaling pathway regulation, and exogenous apoptotic signaling pathway. Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis showed that the differential genes were mainly involved in cell apoptosis and metabolism. ③The mRNA and protein expression levels of several related genes were verified by RT-qPCR and Western blot, which was consistent with the trend of transcriptome sequencing (P<0.01).ConclusionRNA-seq sequencing data analysis showed that total flavonoids from Clerodendranthus Spicatu may induce apoptosis of human renal clear cell carcinoma 786-O cells through signaling pathways such as tumor suppressor protein p53 (p53), mitogen-activated protein kinase (MAPK), and nuclear factor-κB (NF-κB).

      摘要:Objective: To observe the effect of Guipi Decoction on the plasticity of prefrontal cortex neurons in rats with chronic unpredictable mild stress (CUMS)-induced depression, and explore the antidepressant mechanism of Guipi Decoction.MethodNinety healthy male SD rats were subjected to sugar water adaptation, and after establishing the baseline of sugar water preference, they were randomly divided into control group (15 rats) and modeling group (75 rats). Rats in the control group received routine feeding, while rats in the modeling group was subjected to CUMS combined with solitary housing for 3 weeks to establish a depression model. Rats in the modeling group were randomly divided into model group, fluoxetine group, and high, medium, and low dose Guipi Decoction groups, and were administered with pure water, fluoxetine, and corresponding doses of Guipi Decoction by gavage, respectively. After 4 weeks of administration, behavioral tests were conducted on rats in each group, and the expression levels of neuronal nuclei (NeuN) and glycogen synthase kinase-3β (GSK-3β) in the prefrontal cortex of rats in each group were observed using immunofluorescence and Western blot methods.ResultImmunofluorescence and Western blot results showed that the expression of NeuN in the prefrontal cortex of rats in the high dose Guipi Decoction group was significantly higher than that of the model group (P<0.05, P<0.01); Western blot results showed that the expression of GSK-3β in the prefrontal cortex of rats in the high dose Guipi Decoction group was significantly lower than that of the model group (P<0.01).ConclusionGuipi Decoction may exert antidepressant effect by regulating the plasticity of prefrontal cortex neurons in depressed rats.

      摘要:Objective: To explore the mechanism of Wuling Powder combined with Taohong Siwu Decoction in the treatment of heart failure through network pharmacology, molecular docking and experimental verification.MethodsCompound and disease-related databases were searched for network pharmacology predictions. Cytoscape software was used for visualization, DAVID website was used for Gene Ontology (GO) enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway prediction, and molecular docking was performed for core genes of interest to predict their binding degree. The doxorubicin induced H9C2 myocardial cell injury model was used to verify the anti-inflammatory effects of the combination of two formulas(combined formula) and Wuling Powder single formula.Results①Network pharmacological analysis showed that there were 205 common genes and 67 compounds with higher activity between combined formula and heart failure, and the predicted related signaling pathway may be Toll-like receptor (TLR) pathway. ②Molecular docking was carried out between TLR4 gene and major compounds, and the energy was <-5 kcal/mol. ③CCK-8 experiment results showed that the combined formula had a better enhancing effect on H9C2 cell vitality than Wu Ling Powder single formula (P<0.001). ④Western blot experiment results showed that the combined formula had a stronger inhibitory effect on the expression of TLR4 and myeloid differentiation primary-response protein 88 (MyD88) proteins in TLR pathway compared with Wu Ling Powder single formula (P<0.05).ConclusionThe combination of Wu Ling Powder and Taohong Siwu Decoction can effectively resist myocardial cell damage, and its effect is better than that of Wu Ling Powder single formula, and TLR4 signaling pathway plays an important role in it.

      摘要:This article explained Li Dongyuan’s “yin fire” theory based on literature study and clinical practice, and it was believed that the disease of “yin fire” was due to internal injury to diet, fatigue and seven emotions. The “yin fire” was characterized by asthenia fire, with clinical characteristics such as heat manifestitions with internal deficiency, depression and introversion, obvious in the local, and more serious after consumption. Its core pathogenesis was deficiency of qi, deregulation in ascending and descending, and heat depression in the middle jiao. The course of the disease had a tendency from asthenia heat to asthenia cold, and could be divided into qi deficiency yin fire, yin fire with dampness and cold dampness based yin fire. Buzhong Yiqi Decoction, Bupiwei Shengyangqi Xieyinhuo Decoction and Shensheng Fuqi Decoction can be used for the above three stages.

      摘要:Post-cholecystectomy diarrhea is one of the post-cholecystectomy syndromes with a high incidence rate. At present, western medicine treatment is not effective. Traditional Chinese medicine can play an important role in the treatment of post-cholecystectomy diarrhea. This article analyzes the causes of diarrhea due to yang deficiency after cholecystectomy and the mechanism of warming yang therapy for diarrhea after cholecystectomy, in order to provide ideas for clinical treatment.

      摘要:Ginseng, as a valuable traditional Chinese medicine, has a variety of pharmacological activities, and its active ingredient is mainly ginsenoside. Ginsenosides are mainly known as dammarane type tetracyclic triterpenoid saponins,including protopanaxadiol and protopanaxatriol saponins, which are difficult to be absorbed and utilized directly by human body after oral administration. But the scondary ginsenosides or aglycones generated by hydrolysis under the action of intestinal flora can be absorbed and utilized by the human body. Recent studies have shown that protopanaxadiol saponins can be converted into ginsenosides CK,Rh2 and protopanaxadiol (PPD) mainly by intestinal flora, while protopanaxatriol saponins can be converted into ginsenosides Rh1,F1 and PPT by intestinal flora,and the metabolites often show stronger antitumor activity than the prototype ginsenosides. The deglycosylation of ginsenosides by intestinal flora was used as an entry point to sort out their metabolic pathways and summarize the anti-tumor mechanisms of related metabolites,providing a scientific basis for the development and application of ginsenosides in antitumor fields.

      摘要:Diabetic kidney disease (DKD) is a major complication of diabetes, the pathogenesis of which has not yet been fully understood. Studies confirmed that oxidative stress is of great significance to the progression of DKD, and inhibiting oxidative stress can effectively delay the development of DKD and improve the prognosis of DKD. Traditional Chinese medicine (TCM) has the characteristics of multi-target and multi-pathway therapy. In-depth understanding of the regulatory effect of TCM on DKD oxidative stress pathway will be helpful to explore the mechanism of TCM in preventing and treating DKD, and provide a theoretical basis for further conducting more high-quality evidence-based research in the future.
