摘要

目的

建立基于中医体质的老年人肌少症风险预测模型。

方法

将来自社区卫生服务中心的3 519例老年人按7∶3随机分为训练集和验证集。根据Lasso回归和多因素Logistic回归分析中中医体质与肌少症的相关性结果，构建列线图预测模型，采用受试者工作特征（ROC）曲线、Hosmer-Lemeshow校准曲线及决策曲线分析（DCA）评估该模型的预测价值。

结果

社区老年人中，肌少症的患病率为10.29%。中医体质中的阴虚质是老年人肌少症的危险因素，血脂等也是关联因素。训练集和验证集的ROC曲线下面积（AUC）分别为0.704（95%

：0.672~0.736）和0.741（95%

：0.691~0.791），校准曲线和DCA显示模型具有一定的预测价值。

结论

中医体质可作为辅助诊断老年人肌少症的风险因素，可考虑从中医角度调理体质以预防肌少症。

Abstract

Objective： To establish the predictive model for the risk of sarcopenia in the elderly people based on traditional Chinese medicine （TCM） body constitution.

Methods

A total of 3 519 elderly people from community health service center were randomly divided into a training set and a validation set according to a ratio of 7∶3. According to the correlation results between TCM body constitution and sarcopenia in Lasso regression and multivariate Logistic regression analysis， a nomogram predictive model was constructed， and its predictive value was evaluated by Receiver Operating Characteristic （ROC） curve， Hosmer-Lemeshow Calibration Curve and Decision Curve Analysis （DCA）.

Results

The prevalence of sarcopenia in the elderly people from community was 10.29%. Yin deficiency in TCM body constitution was the risk factor of sarcopenia of the elderly people. Blood lipid and other factors were also the related factors. The Area Under the Curve （AUC） of the training set and validation set was 0.704 （95%

： 0.672~0.736） and 0.741 （95%

： 0.691~0.791）， respectively. The calibration curves and DCA showed that the model ha

d a certain predictive value.

Conclusion

TCM body constitution can be used as a risk factor for assisting in the diagnosis of sarcopenia in the elderly， and it can be considered to regulate the body constitution from the perspective of TCM to prevent sarcopenia.
